Community Health Connect

Community Health Connect is a 501(c)(3) nonprofit organization dedicated to enhancing healthcare access for low-income, uninsured men, women, and children in Utah County. The mission focuses on providing quality medical, dental, and mental health care through a network of volunteer providers and community resources.

Mission and Purpose

The primary objective of Community Health Connect is to bridge the gap in healthcare access by coordinating a Volunteer Provider Network. This network consists of healthcare providers who dedicate their time to treating uninsured and low-income residents. History

Established in July 1999, Community Health Connect has been supported by various corporations, nonprofits, and regional foundations. Its fiscal agent is United Way Community Services of Utah County, while administrative duties are handled by Mountainlands Community Health Center.

Program Components

• Community Member Network: This component involves recruiting, training, and supervising volunteers who assist with tasks such as medical translation and navigating healthcare services for new Medicaid or CHIP enrollees.
• Volunteer Provider Network: The organization coordinates healthcare providers who offer their services to uninsured residents.
• Referral Coordination: Referrals are accepted from healthcare providers within the community, including Mountainlands physicians and other local medical centers.
