WE DO THIS BY:-DELIVERING UNBIASED AND CULTURALLY COMPETENT SERVICES TO ALL PATIENTS; -OFFERING COMPASSIONATE, INNOVATIVE, AND USER-FRIENDLY MEDICAL SOLUTIONS; -FORMING COLLABORATIONS AND PARTNERSHIPS WITH OTHER HEALTHCARE PROVIDERS AND STAKEHOLDERS; -PROVIDING OPTIMAL CUSTOMER SERVICE AT ALL TIMES; -ENDEAVORING TO TREAT ALL PATIENTS WITH DIGNITY AND RESPECT REGARDLESS OF ABILITY TO PAY

PatientCentered Medical Home The Wellness Plan Medical Centers is a patientcentered medical home PCMH a model of care where patients engage in a oneonone relationship with their primary care provider who acts as a liaison to further care from an extensive medical team. Community Involvement The Wellness Plan Medical Centers community involvement team acts as a connector for patients and the community to build relationships and provide support.
