Conscience Community Network CCN is a network of innovative and experienced social service providers that collaborates with youth and families to identify and address their unmet needs. CCN uses a strengthbased model of care called Wraparound which brings together the family and their support system to identify their needs strategize how to address their needs and collectively work together to meet their needs. We Believe that Needs represent the root cause of why a behavior or an issue may occur.
