All Aged Care Ltd (formerly OzPol CCAI) is a multicultural nonprofit approved aged care provider with experience providing care and services in Queensland under the Home Care Package program, Commonwealth Home Support Programme, DVA Community Nursing, Community Visitor Scheme and Queensland Health Transition Care. All Aged Care service delivery and philosophy are driven by the quality standards encapsulated in the Accreditation and Community Care Common Standards. All Aged Care focus on genuine community engagement for consumers and reduction of social isolation. All Aged Care address their clients home care needs by focusing on improving access, and establishing, organising, and promoting services and facilities that foster the consumer s mental, physical and social wellbeing. All Aged Care offers the community a suite of Government funded aged care services including; Home Care Packages level 14, allied health, social support, domestic assistance, transport and centrebased day respite. Through provision of services, we actively encourage clients to focus on maintaining independence and living in their own homes for as long as possible.
