Community Health Connections (CHC) is a nonprofit, Federally Qualified Health Center (FQHC) with five locations in North Central Massachusetts offering walk in urgent care, primary family medical and pediatric care, preventative and restorative dental care, oral surgery, behavioral and mental health services for children and adults, substance use disorder treatment, specialty services including optometry eye care, optical shop, acupuncture, nutrition consultations and podiatry. CHC is mission-driven, providing compassionate, high quality health care regardless of income or health insurance status. Our vision is to be the leading healthcare center, committed to eliminating barriers to care and building a healthier tomorrow for the 36 communities we serve.
