The NJHC is a partnership of 140 organizations sharing data and engaging in health improvement planning for collective impact to improve health in North Jersey Mission: Building Capacity to Improve Community HealthThe North Jersey Health Collaborative (NJHC) is multi-sector partnership of community organizations, health care, public health, local government, education, faith groups, and businesses working across sectors to bring about meaningful and sustainable change in the health of our communities. With more than 100 partners in five counties of Northern NJ, workgroups are deploying strategies to target the priority health issues identified in our shared community health needs assessment which includes healthy eating and active living; access to health care and mental health services; prevention and management of obesity, diabetes and hypertension; substance use education and prevention; family caregiver health and support, access to functional transportation; and health literacy. NJHC is a data driven organization, collecting and sharing data and best practices via our website (www.njhealthmatters.org) to inform, monitor and track progress of our collective work. The NJHC's regional scope allows for broad collaboration across county lines in Morris, Passaic, Sussex, Union and Warren Counties, while at the same time we engage grassroots organizations and community members to bring about change in local communities.
