The Choose Healthy Life Black Clergy Action Plan is a sustainable, scalable and transferable approach centered around the Black Church – the oldest and most trusted institutions in the Black community – to address public health disparities. Choose Healthy Life (CHL) is a non-profit faith-based organization that ensures churches the oldest and most trusted institutions in the Black community receive the necessary resources, training, and support to address health disparities by making available health services and delivering community wellness programs in hard-to-reach communities. CHL initially established a network of fifty (50) Black churches in five major cities and grew to 120 churches across 13 states and the District of Columbia. The successful CHL model is centered around the establishment of a full-time faith-based health worker in each church.
