⭐ FEEDBACK FRIDAY ⭐ A huge thank you to our Hospital Discharge Support Worker, Laura for the support she provided to a client, they appreciated the help so much and provided this feedback. Our Hospital Discharge Service supports older adults returning back into their community after a stay in hospital. Our staff provide key support, actively promoting the wellbeing of older people and providing practical and emotional support to allow them to resettle and remain living independently in their own homes, in their own communities.
