Presentation From Data to Home: Digital Patient Selection in HITH RMH@Home Acute partnered with their electronic medical record team to design and implement a computerised clinical decision support system that helps identify potential patients for HITH. Since its introduction, the system has achieved measurable improvements, including an average bed occupancy of 95% and shorter referral times post admission, achieved without additional staffing resources. In this session, the RMH@Home Acute team will outline the development and implementation process, highlight the impact on service delivery, and share practical lessons that may assist other HITH services exploring digital innovation. The HITH Society is pleased to announce that applications are open for the 2025 Janis Coconis Scholarships. More information can be found on our web page (link below) Jan Coconis was one of the first nurses to join the HITH service at The Prince of Wales hospital in Sydney, Australia. During her career Jan co-authored many seminal papers on HITH and was a founding executive member of the HITH Society. Sadly, Jan passed away in 2012. The HITH Society has established the Janis Coconis scholarship in recognition of Jan’s outstanding contribution over many years to the field of HITH in Australia. This scholarship is awarded to provide financial assistance to up to two Registered Nurses and up to two Allied Health Professionals working within the HITH setting in Australia or New Zealand to attend the HITH Society Australasia's Annual Scientific Conference.
