Brisbane North PHN is pleased to share our Equity and Access Framework, a pivotal step towards building a more inclusive and fair healthcare system across the North Brisbane and Moreton Bay region. The Framework identifies four aims for our PHN as we embed it into our core business: ➡️ Drive equity through our organisational strategy and be accountable to our community through governance arrangements 👂 Listen, understand and respond to the diverse needs of our community ♻️ Embed access and equity in our commissioning processes 🤝 Prioritise cultural capability at an organisational level, reflecting the diversity of the communities we support and connected to our partners in the local health system. 🗣 "As a care finder, I see the hardship firsthand, and having a resource that is practical, concise, compassionate, and reliable makes all the difference." In response to the growing housing crisis which is increasingly impacting vulnerable older people across our region, Brisbane North PHN and Footprints Community have today launched the Care Finder Housing and Homelessness Guide. 🔍 The guide, developed based on care finder feedback and consultation, will "help to fill any knowledge gaps and provide care finders with the tools, information and confidence they need to help their clients navigate a complex housing environment during what is a highly stressful time for them," said Footprints care finder Senior Manager, Justin McNellie. 🖱️ Read more via our Latest News: https://bit.ly/4e8P9P2.
